Booking Your Appointment

Prior to booking your appointment, you will need to:

When booking your appointment, please provide us with:

  • Enquire with Vitality to obtain an authorisation number
  • Find out if you have any excess that you will need to pay
  • Your membership number
  • The authorisation code
  • Your reason for visiting the clinic
For subsequent visits, please check with Vitality if you will need another authorisation code. If Vitality fails to cover your consultation fees, we will send you an invoice to be paid within 30 days.
